Diabetes Mellitus and Hypertension are chronic diseases with high prevalence and serious complications if not properly controlled. Low public health literacy regarding early detection of complications is one of the factors contributing to the increased risk of disease complications. This community service activity aimed to improve the knowledge and understanding of Chronic Disease Management Program (Prolanis) participants regarding the early detection of diabetes mellitus and hypertension complications at Nan Balimo Community Health Center. The activity was conducted through educational socialization using interactive lectures and discussions, followed by group morning exercise as promotive and preventive efforts. The program involved 30 Prolanis participants and was supported by 5 healthcare workers consisting of a doctor, a midwife, a public health worker, and nurses. The results showed that participants demonstrated high enthusiasm during the educational session and were able to understand the early signs and symptoms of diabetes mellitus and hypertension complications after receiving the education. In addition, participants actively participated in the morning exercise session as an implementation of healthy lifestyle behavior. This activity had a positive impact on improving the health literacy of Prolanis participants regarding the prevention of chronic disease complications. Therefore, health education integrated with physical activity should be carried out continuously to improve the quality of life of patients with chronic diseases.
